What happened

An org runs at most one policy version in shadow. Accepting a suggestion creates a new version and runs it in shadow, and another version is already running there. The suggestion goes back to open; nothing changed.

What to do

Promote or deactivate the running shadow first, or accept again with "replace_shadow": true (--replace-shadow in the CLI) to replace it. Replacing seals a deed that names the version it replaced.

The message is for a person and can get more exact over time: branch on type and code, never on the message.
