The decision carried a context_digest that isn’t the approval’s. Every decision names the exact context snapshot the person reviewed; a page showing something else can’t be used to approve.

What to do

Reload the approval, review what it shows now, and decide again.

The message is for a person and can get more exact over time: branch on type and code, never on the message.
