What happened

You countersigned a four-eyes approval (a rule with required: 2 or more) and tried to countersign it again. Four-eyes counts distinct people, so a second vote from the same person never counts.

What to do

Ask another approver of the rule to countersign. The action runs when the last required person approves.

The message is for a person and can get more exact over time: branch on type and code, never on the message.
